    Description

    This study will examine a novel model of interdisciplinary care in RACFs, with an on-site pharmacist working alongside the nursing staff to form an interdisciplinary team to improve medicine management and prevent medication-related adverse health outcomes. We propose conducting a study which includes a randomised cluster-controlled trial in the ACT to examine the impact of this model of care on the appropriateness of medication use in RACFs in the ACT. A mixed methods design will be used to determine the acceptability and ongoing sustainability of the new model using the Reach Effectiveness Adoption Implementation Maintenance Framework (RE-AIM) (8) . Additionally, the cost-consequences of the model will be determined.
