Social referral initiatives: A transformative approach to improve thehealth and well-being of Aboriginal and Torres Strait Islander peoplewith chronic conditions

IV25 '2024 Investigator Grant' - 2033811 - EL1 -- Social referral initiatives are a transformative approach for linking Aboriginal and Torres Strait Islander people withchronic conditions to a range of various services and supports. Connecting people from primary care to services andsupports will address peoples unmet holistic needs. In the proposed research, I will evaluate how people with chronicconditions can be best supported to access services and participate in activities to manage their conditions andimprove their quality of life.
